Senior Care Navigator LA offers a wide range of services to help older adults and their families, from assessments to treatment plans to ongoing care management. For services we can’t provide ourselves, such as elder legal advice or estate planning, we offer referrals from our select network of trusted experts and professional service providers, providing introductions and warm hand-offs to ensure the highest quality of consistent, attentive, and uninterrupted care.

Our services include:

• Phone or video consultations

• Assessment of client (in home or facility)

• Counseling

• Interviewing and selecting caregiver agencies or individuals; ongoing coordination of these services

• Making and coordinating medical appointments

• Attending medical appointments and providing follow-up reports to other care providers

• 24-hour on-call services for emergencies such as meeting client at hospital

• Transitioning care for clients moving from independent to assisted living or from hospital stay to home

Additional services:

• Consulting with estate trustees to determine optimal care solutions for a surviving spouse or partner

• Elder mediation for families seeking to gain alignment on decisions for parents or older family members

• Travel assistance

  • Arrange and coordinate travel plans

  • Accompany a client to visit friends or family members

  • Accompany a client who is moving out of the area
