 What We Do

Senior Care Navigator LA was founded with the belief that we all deserve the highest quality of life possible as we age. For more than 25 years, we have dedicated ourselves to providing expert care and attention to the needs of older adults. We understand how confusing and overwhelming the aging process can be — for both the individual and their family — and our mission is to bring older adults and their caregivers relief and peace of mind.

We help clients and families throughout the greater Los Angeles area in a wide variety of capacities, providing on-site guidance and support and adding additional services as needed. We work with clients in their own homes, in assisted living situations, communities, and in medical facilities. We also help with transitions between home and hospital stays. We always seek to work within the framework most comfortable for the client and family, taking into account that each situation has a unique set of personalities, challenges, and opportunities.

As part of our mission to bring older adults and their caregivers relief and peace of mind, we offer 50-minute video or phone consultations to caregivers in Los Angeles or across the country, billed at $185 per consultation. Consultations can be scheduled on a regular or as-needed basis.

Consultations are quick and effective starting points for longer-term care. They are useful for clients seeking help for a variety of reasons, including:

  • Time-sensitive care needs or questions

  • Providing support and resources for non-local caregivers and family members

  • Planning for the needs of an aging adult who will need help in the future

  • Identifying resources and treatment options

  • Creating a care plan

  • Conducting family meetings

Barbara has devoted her long and rich career in geriatric care to helping people age with grace, dignity, and the best possible professional care. Known for her work in dementia, Barbara is considered a pioneer in the field, providing expert support and caregiving services to older adults and their families in the Los Angeles area and beyond for more than 30 years.

For the past decade, Barbara has led caregiver support groups for Alzheimer’s LA and UCLA’s Easton Center for Alzheimer’s Disease Research, including one for caregivers of persons with Lewy Body Dementia. She also serves on the advisory board of OPICA Adult Day Care Services, Los Angeles’s first Adult Day Program & Counseling Center serving people with dementia and their families.

Wendy’s passion for her work as a geriatric care manager was borne out of her own personal experience of dealing with her mother's battle with pancreatic cancer. As a result of this experience, she has made it her life's mission to help others. Prior to joining Senior Care Navigator LA, Wendy was the managing director of Leeza's Care Connection, a non-profit organization (in collaboration with Providence St. Joseph Hospital) which helps families cope with dementia or Alzheimer's Disease.

Deborah casillas

  • Clinical nurse

Deborah is a gifted clinical nurse who has worked in geriatric care for decades. She has experience as a medical coordinator and mediator with VITAS Healthcare and as a healthcare provider for patients in hospice and patients in acute respiratory distress.

 

 Services

Senior Care Navigator LA offers a wide range of services to help older adults and their families, from assessments to treatment plans to ongoing care management. For services we can’t provide ourselves, such as elder legal advice or estate planning, we offer referrals from our select network of trusted experts and professional service providers, providing introductions and warm hand-offs to ensure the highest quality of consistent, attentive, and uninterrupted care.

Our services include:

  • Phone or video consultations

  • Assessment of client (in home or facility)

  • Counseling

  • Interviewing and selecting caregiver agencies or individuals; ongoing coordination of these services

  • Making and coordinating medical appointments

  • Attending medical appointments and providing follow-up reports to other care providers

  • 24-hour on-call services for emergencies such as meeting client at hospital

  • Transitioning care for clients moving from independent to assisted living or from hospital stay to home

Additional services:

  • Consulting with estate trustees to determine optimal care solutions for a surviving spouse or partner

  • Elder mediation for families seeking to gain alignment on decisions for parents or older family members

  • Travel assistance

    • Arrange and coordinate travel plans

    • Accompany a client to visit friends or family members

    • Accompany a client who is moving out of the area

 OPICA Adult Day Program & Counseling Center: www.opica.org

Full-day programs for people with dementia including art, music and brain training for early stage dementia (for a daily fee). They also offer counseling to caregivers and caregiver support groups. During COVID-19, they offer a series of hour-long virtual programs on zoom.

A senior community resource located in Santa Monica. Services include an Adult Day Service program for people with dementia, socialization programs and activities, caregiver support groups, educational seminars, and meals. Most activities are virtual during COVID – call for details: 310-394-9871.

A non-profit organization that serves the LA area, providing resources for all ages including seniors. JFSLA provides Holocaust Survivor services, case management, communal dining in locations around the city, home delivered meals and activities.  Note: At this time all services are online – check website for updates.

Provides services for seniors all day, including meals, activities, physical and occupational therapies. Also provides door to door transportation.  Accepts Medi-Cal. Note: During COVID-19 services are limited so call for information: 213-250-9191.
