Our team works closely with older adults and their families to assess individual needs, create personalized care plans, and coordinate a range of services to support well-being and quality of life.
Step 1 - Geriatric Assessment
Our assessment is a comprehensive evaluation of an older adult’s health, living situation, and support system, conducted in the home, facility, or medical setting. It serves as the foundation for our services, covering key areas such as:
• Health status
• Functional and cognitive abilities
• Emotional and social needs
• Safety and environment
• Caregiver support
• Financial and community resources
Step 2 - CARE PLAN
Based on our assessment, we develop a personalized care plan with specific recommendations. A care plan can be used as a starting place for ongoing care management or as a roadmap for older adults and their caregivers to implement on their own.
Step 3 - CARE MANAGEMENT
Recognizing that urgent needs often arise, we offer flexible and consistent care coordination and oversight of a wide range of services, including:
• Monitoring and adjusting care levels and settings
• Guiding Assisted Living placement
• Implementing care and mobile services in the home
• Managing medical appointments, medications, tests, and transitions
• Offering dementia coaching and facilitating communication among family members to address emotional and practical challenges
• Coordinating and offering travel assistance
• Providing 24/7 responsiveness for client emergencies
• Referring to our trusted network of specialists for auxiliary services such as legal, technology, or estate planning
